I was just at the post office on Thursday to submit my application form.
To change your passport in your married name, you need to complete the 'new passport application' form for Adult (this includes getting someone to fill in the guarantor section and sign your photo and showing all your documents).
Documents you will need to take can include.......
* Australian birth certificate or Australian citizenship certificate
* marriage certificate
* change of name certificate
* birth certificate (if not born in Australia)
* your current Australian passport
* drivers licence
* medicare card
The documents above are the ones I took to the passport 'interview'. Read the application form thoroughly. When I was at the post office a few weeks ago, there was someone who was doing their passport and the applicant was told she would have to come back because she didn't bring all her documents and also the photos were not up to the specifications listed on the application form
Definitely take originals (better be safe than sorry). The application form says to take the original and a photocopy (most post offices should have a photocopier.....).
